Job summary

South Middlesex Opportunity Council is seeking a Case Manager to provide one-on-one support to veterans, helping them develop service plans, pursue employment, budgeting, nutrition, health care, mental health treatment and substance use counseling.

The aim is to aid each person in obtaining and sustaining a permanent housing placement, typically managing a caseload of 15 veterans. You will work closely with care teams, document meetings, and advocate at administrative hearings when required.

Qualifications

• Bachelor's Degree or a minimum of three years experience in Human Services or related field.
• Sensitivity to low-income families of diverse backgrounds.
• Ability to work independently.
• Good written communication skills.
• Valid driver's license and ability to meet our insurance standards.
• Assessment, advocacy and case management skills.
• Bilingual preferred.

Responsibilities

• Work 1:1 with new placements to ensure units are ready and equipped prior to arrival, greet the placement and conduct tour.
• Complete intake and needs assessment covering income/employment/education, budgeting, behavioral health, food, legal issues, health, daily living skills.
• Meet weekly to monitor the re-housing plan and document all meetings.
• Perform safety inspections on units and use required forms.
• Coordinate with care team to identify strengths and address barriers.
• Support clients with scheduling meetings around work; pre-authorized by supervisor.
• Coordinate services and report adherence to shelter rules.
