Job summary

Veteran Pathways of New England seeks a Case Manager to deliver housing stabilization and comprehensive case management for Veterans across Massachusetts. The role operates on-site at Devens and in the field within the assigned service area, with travel as needed to support clients.

You will conduct assessments, develop Housing Stability Plans, administer TFA, and connect clients with VA and local benefits, while maintaining thorough documentation and reporting.

Qualifications

• Associate or bachelor’s degree in Social Work, Human Services, Public Health, or related field.
• 1–3 years of experience in case management or related human services.
• Strong organizational and communication skills; ability to manage a caseload.

Responsibilities

• Provide housing stabilization and case management services.
• Develop assessments and Housing Stability Plans tailored to client needs.
• Administer Temporary Financial Assistance (TFA) and connect clients with VA, state, and local benefits.
• Maintain accurate client files and documentation in a compliance-driven environment.
• Engage with Veterans and service members in the community.
