Job summary

CLEAR PATH FOR VETERANS seeks a dedicated Case Manager to support Veterans across Cape Cod and Suffolk County, Massachusetts. This role involves developing individualized stabilization plans, conducting outreach, and providing advocacy for Veterans and their families.

Successful candidates will have 1–3 years of relevant experience and strong organizational and communication skills. This field-based position requires reliable transportation and offers meaningful work with a comprehensive benefits package.

Qualifications

• 1–3 years of experience in case management, housing support, or social services.
• Strong organizational, communication, and problem-solving skills.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Develop individualized stabilization plans for Veterans.
• Conduct outreach and connect with Veterans in need.
• Maintain accurate client files and documentation.
