Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or related field.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Certification in Substance Abuse Counseling preferred.

Responsibilities

• Assesses veterans referred to the program, addressing housing, employment, education/training, finances, and health needs.
• Coordinates care with VA personnel and community partners for eligibility and services.
• Facilitates intake, compiles client files, and ensures documentation is complete.
• Develops Individual Action Plans (IAPs) with clients.
• Provides guidance to junior staff on IAPs and case management.
• Conducts crisis intervention and maintains confidential files in HIPAA-compliant manner.
• Completes timely discharge summaries and conducts home visits to support health and welfare.
• Documents progress in DAP format and manages HMIS data entry.
