Job summary

Vincentian CARES in San Juan, PR is seeking a Case Manager III to assess, plan, and advocate for veterans and their families under the Supportive Services for Veteran Families program. You will manage caseloads and coordinate housing goals based on clients’ needs and preferences.

The role requires a bachelor’s degree in social work or related field and a minimum of two years’ experience serving homeless or at-risk populations, with strong communication and documentation skills.

Qualifications

• Speak, write and understand English; ability to communicate with diverse populations.

Responsibilities

• Provide crisis intervention for very low-income, homeless veterans.
• Assess strengths, needs, abilities to develop housing goals.
• Design and carry out a Housing Stability Plan for each household.
• Monitor progress daily and document progress toward goals and services.
• Enforce program rules to ensure compliance with regulations.
• Provide access to services and community resources as needed.
• Facilitate move to transitional or permanent housing when appropriate.
• Conduct home visits as needed.
• Determine Eligibility and process Temporary Financial Assistance.
