Job summary

U.S. VETS is hiring a Case Manager III in the United States to provide trauma-informed, strengths-based support to veterans facing housing instability and mental health challenges.

This advanced role leads day-to-day coordination of client services and mentors junior staff. You will assess clients, create IAPs, coordinate with the VA and community resources, conduct home visits, facilitate groups, and ensure thorough documentation.

Qualifications

• Master’s Degree in Social Work, Psychology, Counseling, or related field.
• Two years of experience in social services or related field preferred.
• Experience working with homeless and/or veterans preferred.
• Certification in Substance Abuse Counseling preferred.
• Experience facilitating support groups preferred.
• Strong oral and written communication skills.
• Professional manner and appearance.
• Ability to work independently and within a team.
• Valid driver’s license required.

Responsibilities

• Assess clients and develop individualized service plans (IAPs) addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs.
• Coordinate services with the VA, landlords, employers, treatment providers, and community resources.
• Provide regular one-on-one meetings with clients and conduct home visits (project-based and scattered-site housing).
• Facilitate life skills groups and/or therapeutic groups tailored to veteran populations.
• Lead and support case conferencing and mentor Case Manager I & II team members.
• Assist with crisis intervention, urgent client matters, and after-hours support as needed.
• Document all services and critical incidents in the MCR and other electronic client record systems.
• Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.
• Supervise interns and participate in staff onboarding and training.
