Job summary

Endeavors is seeking a compassionate Veteran Support Case Manager to work in Texas, delivering client-centered case management to Veterans and their families who are experiencing homelessness or at risk. You will help participants achieve housing stability and an improved quality of life.

The role requires advocacy, outreach to shelters and community partners, and coordination of housing, utilities, mental health, and other benefits, with intake assessments and service planning to support

Qualifications

• Bachelor’s Degree in Social Work, Sociology, Psychology or related field.
• 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment.
• LMSW, LBSW, LMFT preferred; Driver’s License with clean record.

Responsibilities

• Advocate for Veterans and facilitate access to housing, utilities, mental health services, medical care and other benefits.
• Conduct initial screenings to determine eligibility and complete intake assessments and service plans.
• Coordinate, monitor and document services to ensure progress toward goals; maintain client files per policy.
• Prepare reports and contribute to program evaluation; collaborate with supervisors and community partners.
• Engage in community outreach to shelters, encampments, and stand-down events; present to organizations.
