Volunteers of America Mid-States (VOA) is seeking a Service]S SVF Case Manager to support veterans and families at risk of homelessness. The role focuses on assessing needs, linking to community resources, and coordinating with VA and community providers to promote housing stability and self-sufficiency.

• Bachelor’s degree in social work or related field or 3 years of relevant experience for veterans.
• Ability to assess family needs and access local resources.
• Advocacy, counseling, and collaboration with local service providers.

Responsabilidades

• Provide outreach to veterans and families to increase program awareness.
• Develop and monitor individualized case plans and follow up.
• Coordinate with HUD-VASH and SSVF teams and community service providers.
• Assist participants in obtaining VA benefits and other public benefits.
• Enter required documentation promptly and maintain HMIS records.
• Advocate for veterans and maintain culturally sensitive practices.
• Participate in CTI training and PQI duties as assigned.
