Qualifications

Responsibilities

• Provide outreach to recruit veteran families for the SSVF program.
• Perform assessments and develop/monitor case plans.
• Establish linkages with local agencies and service providers.
• Educate participants on supportive services and their rights.
• Complete required documentation within 48 hours.
