Qualifications

• Experience in case management, advocacy, and connecting clients to resources.
• Ability to travel between served counties up to 70% of the time.

Responsibilities

• Connect Veterans with benefits and SOAR applications.
• Develop and monitor case plans; follow up with participants.
• Link with agencies, providers, and VA services in the area.
• Coordinate with career centers and job centers for employment education.
• Educate on services, rights, and resources; document progress within HMIS.
