Responsibilities

  • Community Outreach: Conduct outreach at shelters and community locations across Albuquerque to connect with eligible veterans.
  • Intake & Assessment: Evaluate eligibility and identify employment barriers, housing needs, and support requirements.
  • Individualized Planning: Build an Employment Plan with goals, resume building, and interview prep steps.
  • Service Coordination: Link veterans to VA clinics, housing agencies, and treatment programs.
  • Job Placement & Retention: Assist with local job searches, coordinate training, and maintain contact for up to nine months.
  • Data & Compliance: Document client interactions and outcomes in HMIS and other systems.
  • Provides client status updates and discharge information.
  • Transport clients as needed to support program success.
  • Participates in case staffing and other duties as assigned.

Position Summary:

This role focuses on helping veterans experiencing or at risk of homelessness overcome barriers to employment and secure stable housing. You will act as a primary advocate, guiding veterans from initial intake through skill training, job placement, and long-term career stabilization.

  • Community Outreach: Conduct active outreach at local shelters, standard Stand Down events, and community locations across Albuquerque to connect with eligible veterans.
  • Intake & Assessment: Evaluate program eligibility and identify specific employment barriers, housing needs, and clinical or mental health support requirements.
  • Individualized Planning: Work one-on-one with veterans to build an Individualized Employment Plan (IEP) that outlines specific career goals, resume building, and interview preparation steps.
  • Service Coordination: Connect veterans to critical local support systems, including VA medical clinics, local housing agencies, and substance use treatment programs.
  • Job Placement & Retention: Assist participants with local job searches, coordinate vocational training, and maintain continuous contact for up to nine months after placement to ensure job retention.
  • Data & Compliance: Document every client interaction, milestone, and financial assistance voucher within federal tracking databases and local systems like the Homeless Management Information System (HMIS).
  • Provides client status updates and discharge information.
  • Transport clients as needed and required to ensure clients’ success in Heading Homes programs.
  • Participates in case staffing.
  • Other duties as assigned for optimal client support.

Qualifications:

To perform this job successfully, an individual must have:

  • Education: Bachelor’s degree in Social Work , Psychology, Counseling, or Human Services is preferred. Direct professional experience working with veteran care or homeless populations may substitute for a degree.
  • Experience: Minimum of 1–2 years in human services, career counseling, or case management. Experience working directly with military veterans, justice-involved individuals, or vulnerable adults is highly preferred.
  • Local Knowledge: Strong familiarity with Albuquerque-area community resources, transit options, and veteran-specific aid networks.
  • Technical Skills: Proficiency with Microsoft Office suite and electronic case management tracking systems.
  • Screening Requirements: Must possess a valid New Mexico Driver's License, a clean driving record, reliable personal transportation for field outreach, and pass a comprehensive background check.

Other Requirements

  • Reliable transportation.
  • Ability to navigate stairs, ladders, ramps, and uneven terrain.
  • Lifting a maximum of 25 pounds occasionally and frequently lifting or carrying objects weighing up to 10 pounds.
  • Must become CPR/First Aid trained within 90 days of hire.
