Job summary

Goodwill Industries of New Mexico is seeking a full-time Case Manager for the Homeless Veterans Reintegration Program (HVRP) in the Albuquerque area. You will provide case management, advocacy, and crisis support to veterans, documenting progress and coordinating services with community partners.

The role requires travel within the Albuquerque metro and surrounding areas, a strong focus on client-centered planning, and adherence to HIPAA and program guidelines.

Qualifications

• Bachelor’s Degree in social services or related field preferred.
• One to three years’ work experience in case management or social work preferred.
• CCM, CRC or MSW preferred.
• Valid New Mexico driver’s license, liability insurance and clean driving record.

Responsibilities

• Provide case management services to participants via service coordination, advocacy, evaluation, assessment and crisis management.
• Contact participants and provide case management to assigned caseload according to program guidelines.
• Build collaborative relationships with community agencies and maintain confidential case records.
• Complete documentation, budgeting, reimbursements and required reporting on time.
• Ensure HIPAA compliance and safety of persons served; travel within Albuquerque area and state as needed.
