Job summary

Veterans Community Project (VCP) is expanding nationwide to serve more Veterans, providing wraparound services and housing support. The Case Manager will engage Veterans at risk of homelessness, develop housing and stability plans, and connect them to resources while ensuring consistent, compassionate support.

The role emphasizes direct engagement, coordination with housing providers, and follow-up to sustain progress and secure stable housing for Veterans in need.

Qualifications

• Two years’ experience in social services, case management, or Veteran services preferred.
• Knowledge of housing programs and Veterans’ benefits.
• Experience with homelessness, mental health, or substance use challenges preferred.
• Excellent communication and documentation skills.

Responsibilities

• Engage Veterans, assess goals, and provide direct support and referrals.
• Develop case plans and coordinate housing opportunities and follow-up.
• Coordinate emergency financial assistance and ensure plan follow-through.
• Maintain accurate client records and required reporting.
• Collaborate with partners and build community resources.
• Support safe residential environments and property oversight.
