Endeavors Inc. is seeking a dedicated Case Manager to support Veterans and their families who are homeless or at risk. You will advocate for housing, utility, and community services, while building a resource database and coordinating care plans.

The role requires at least 1 year of case management and strong customer service experience, with travel up to 30% and a valid driver’s license. LMSW/LBSW/LMFT are preferred credentials.

Qualifications

• Bachelor’s Degree required in Social Work, Sociology, Psychology or related field.
• 1+ years of case management experience; 3+ years preferred.
• 2+ years in a customer service-focused environment.
• Experience with homeless, low-income, veterans & their families is a strong plus.
• Driver’s license with a clear record; LMSW/LBSW/LMFT preferred.
• Daily use of a vehicle; up to 30% travel to county and surrounding areas.

Responsibilities

• Advocate for and facilitate Veterans’ access to housing, utilities, other community services and benefits.
• Build a database of community resources and provide referrals as needed.
• Conduct initial intake meetings and evaluate needs; complete service plans with housing and related needs.
• Coordinate and monitor services with comprehensive tracking of Veteran activities.
• Maintain detailed case notes and client files; prepare reports for senior staff.
• Coordinate payments with Financial Assistance Coordinator and inform Veterans of actions.
• Attend staff meetings and staff caseload; participate in community groups and presentations.
