Qualifications

  • Bachelor’s Degree in Social Work, Sociology, Psychology or related field.
  • 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment.
  • LMSW, LBSW, LMFT preferred. Driver’s License with clear record.
  • Vehicle required; up to 50% travel within the county and surrounding areas.

  • Ensure timely payments to landlords and vendors; communicate status of assistance and disbursements.
  • Meet with leadership to review caseloads and support program growth; participate in community coalitions.

JOB PURPOSE

Provide comprehensive case management to Veterans and their families who are homeless or at risk of homelessness. To provide quality of life and maintain the highest possible ability to function within the community.

ESSENTIAL JOB RESPONSIBILITIES
  • Advocate for Veterans and facilitate access to housing assistance, utility assistance, healthcare, transportation, income support, and other community resources and benefits. Develop and maintain a comprehensive database of community resources and referral partners, build collaborative relationships with service providers, and coordinate referrals and linkage to services that address identified needs and support housing stability, self-sufficiency, and overall well-being.
  • Provide intensive, hands-on support and advocacy for Veterans experiencing chronic homelessness who may face significant barriers to self-sufficiency, including limited life skills, behavioral health challenges, or difficulty navigating complex systems. Assist Veterans with completing housing applications, benefit applications, and other required paperwork, and when necessary, accompany and transport participants to appointments, housing viewings, and service provider meetings based on individual needs, abilities, and level of support required
  • Conduct home visits and community-based follow-up contacts to monitor housing stability, assess ongoing needs, reinforce progress toward service plan goals, and provide supportive case management services
  • Assess the needs, strengths, barriers, and goals of Veterans and their families; develop individualized housing and service plans in collaboration with participants, establish measurable action steps to address identified needs, and provide ongoing case management support to promote housing stability and self-sufficiency. Regularly review and update housing and service plans to reflect progress, completed objectives, changing circumstances, and emerging needs to ensure services remain aligned with participant
  • Assess the needs, strengths, barriers, and goals of Veterans and their families; develop individualized housing and service plans in collaboration with participants, establish measurable action steps to address identified needs, and provide ongoing case management support to promote housing stability and self-sufficiency. Regularly review and update housing and service plans to reflect progress, completed objectives, changing circumstances, and emerging needs to ensure services remain aligned with participant
  • Coordinate, monitor, and document service delivery to ensure progress toward housing and service plan goals, including comprehensive tracking of Veteran activities, referrals, outcomes, and case management interactions; conduct and document housing inspections as required; assist Veterans in obtaining identification cards, DD214s, birth certificates, Social Security cards, and other essential documents needed to access benefits, healthcare, employment, community resources, and permanent housing.
  • Coordinate with the Site Supervisor and Financial Assistance Coordinator to facilitate timely payments to landlords, utility providers, and other approved third-party vendors. Maintain regular communication with Veterans regarding the status of requested assistance, payment approvals, and disbursements, ensuring they remain informed of actions taken and any additional documentation or follow-up required.
  • Coordinate with the Lead Case Manager and Financial Assistance Coordinator to facilitate timely payments to landlords, utility providers, and other approved third-party vendors. Maintain regular communication with Veterans regarding the status of requested assistance, payment approvals, and disbursements, ensuring they remain informed of actions taken and any additional documentation or follow-up required.
  • Meet regularly with the Lead Case Manager to review caseloads, discuss client progress and barriers, coordinate service strategies, and ensure effective case management. Provide ongoing feedback and recommendations to the Lead Case Manager and Program Manager to support program growth, quality improvement, and successful client outcomes.
  • Actively participate in monthly Veteran community coalitions, networking groups, and stakeholder meetings to strengthen partnerships and coordinate services for Veterans. Deliver presentations and program information to community organizations and partners throughout the service areas.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS
  • EDUCATION: Bachelor’s Degree in Social Work, Sociology, Psychology or related field.
  • EXPERIENCE: 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment. Experience with homeless, low income, veterans & their families a strong plus.
  • ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
  • LICENSES: LMSW, LBSW, LMFT preferred. Driver’s License with clear record.
  • VEHICLE: Must have daily use of a vehicle without prior notice. Up to 50% travel within the county and surrounding areas.
  • OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
