Job summary

The position is full-time, 40 hours per week, with an on-site schedule from 8:00 AM to 4:00 PM, Monday to Friday. A valid Ohio driver’s license and background check are required to support field outreach.

Qualifications

• High School Diploma or equivalent required; college coursework preferred.
• Experience with homelessness, domestic violence, chemical dependency, mental health preferred; veterans experience preferred.
• Demonstrated organizational, interpersonal, and communication skills.
• Willingness to learn and apply best practice models as appropriate.
• Must be able to meet agency background check requirements (Ohio BCI).

Responsibilities

• Answers veterans' support line to screen and provide advocacy and referrals to resources.
• Travels to community locations to conduct outreach for homeless and at-risk veterans.
• Screens veteran households by phone or in-person to determine program eligibility.
• Documents and coordinates meetings; coordinates outreach activities.
• Collaborates with veteran program staff to facilitate program placement.
• Works with the case management team to ensure seamless transition from outreach to participation.
• Participates in Coordinating Team meetings to ensure consistency and effectiveness.
• Provides crisis intervention as appropriate; participates in community events promoting services.
• Complies with agency policies, COA regulations, and state/federal requirements.
