Qualificações

• One year of experience in social services.
• Employment at U.S.VETS for at least three months may substitute for one year of experience.
• Strong oral, written, and communication skills.

Responsabilidades

• Conduct Community Outreach: Visit shelters, treatment facilities, soup kitchens, and community locations to connect with veterans.
• Build Relationships: Establish rapport with veterans and community partners to encourage engagement in services.
• Coordinate Referrals: Schedule screenings, transportation, and referrals for veterans entering programs.
• Track Client Information: Maintain databases and documentation related to outreach activities and referrals.
• Support Program Events: Assist with transportation and coordination for outreach activities and special events.
• Collaborate with Providers: Work with external agencies and partners to collect required documentation and coordinate services.
• Produce Reports: Maintain outreach logs and weekly reports of veterans engaged by the outreach team.
• Participate in Team Meetings: Attend outreach meetings and collaborate with staff across programs.
