Job summary

Axess Family Services, Inc. seeks an Outreach Specialist to answer the veterans' support line and conduct outreach for homeless and at-risk veterans across multiple counties.

The role is full-time, 40 hours per week, with a $40,000 yearly salary and a schedule of 8:00 AM–4:00 PM, Monday through Friday. The incumbent will screen for program eligibility, document meetings, coordinate with veteran program staff, and participate in team meetings to ensure consistent, high-quality service delivery.

Qualifications

• High School Diploma or equivalent required; college coursework preferred.
• Experience with homelessness, domestic violence, chemical dependency, or mental health is preferred.
• Demonstrated organizational and communication skills; ability to learn and apply best practices.

Responsibilities

• Answers veteran support line and conducts outreach for homeless and at-risk veterans.
• Travels to community locations to ensure effective local outreach.
• Screens for program eligibility and coordinates referrals and advocacy.
• Documents coordination meetings and provides outreach.
• Coordinates with veteran program staff to facilitate program placement.
• Works with the case management team to ensure seamless service delivery.
• Participates in regular coordinating team meetings for program consistency.
• Provides crisis intervention as appropriate.
• Participates in community events promoting veteran services.
• Complies with agency policies, COA regulations, and required certifications.
