Job summary

InteCare is seeking a dedicated Veterans Services Intake Coordinator to enroll Veterans into the SSVF program, collect required documents, and ensure HMIS data accuracy. The role involves connecting Veterans to their assigned case managers and conducting initial orientation about program rights and responsibilities.

The position emphasizes outreach, eligibility screening, and maintaining accurate housing stability plan data, with a focus on serving Veterans and their families in the Indianapolis

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in health care, mental health care or related field preferred.
• Master’s degree preferred.

Responsibilities

• Enroll Veterans and enter documentation into HMIS; connect Veterans to their case manager.
• Collect all documentation for SSVF enrollment.
• Ensure documentation compliance with SSVF enrollment standards.
• Complete Homeless Prevention Screening for HP Veterans.
