Job summary

Rocky Mountain Human Services is seeking a Veteran Care Coordinator to provide in-person, trauma-informed case management for Veterans experiencing homelessness or at risk. You will support housing stability through collaboration with the VA SSVF program, manage documentation, and connect clients to housing and community resources.

The role requires regional travel within Colorado, strong organizational skills, and commitment to confidentiality.

Qualifications

• 1-2 years of experience with homeless individuals or veteran populations.
• Bachelor’s degree preferred in social work, social sciences, or related field.
• Valid driver’s license and proof of insurance.
• Prior case management experience.

Responsibilities

• Assess veteran needs and eligibility for services.
• Coordinate with veterans, families and community agencies to facilitate services.
• Meet/work with veterans in various settings.
• Attend staff, team, and department meetings.
• Develop and maintain client case records, plans and reports.
• Drive personal or company vehicle.
• Lift and/or carry 20 lbs.
• Maintain prompt and regular attendance.
