Qualifications

• Bachelor's degree in Social Work or related field.

Responsibilities

• Conduct intakes and housing navigation to assist veterans at imminent risk or experiencing homelessness.
• Develop Housing Stability Plans and provide case management to help veterans achieve stability goals.
• Collaborate with Health Care Navigator and Employment Specialist to connect veterans with additional services.
• Maintain timely and accurate documentation of services in HMIS and client records.
• Represent the program in community meetings, case conferences, and outreach in the Panhandle region.
