Job summary

The Salvation Army Kroc Center in Pittsburg, KS is seeking a Case Manager for the Supportive Services for Veteran Families (SSVF) program. You will provide case management and connect veterans to rapid resolution, rehousing and homeless prevention services, coordinating with VA and community partners.

Responsibilities include intake, assessment, housing stability planning, advocacy, and data entry into HMIS. This is a full-time role, with staff training and travel as needed.

Qualifications

• BA/BS in social work preferred.
• 2–5 years of relevant experience preferred.
• Experience with motivational interviewing, crisis intervention and trauma-informed approaches.

Responsibilities

• Provide case management and supportive services to Veteran households under SSVF.
• Coordinate rapid resolution, rapid rehousing and homeless prevention efforts.
• Engage in outreach, intake, assessment and housing stability planning.
• Liaise with VA, CoC, and community partners to meet participant needs.
• Maintain case documentation and HMIS data entry.
