Job summary

The Salvation Army USA Western Territory seeks a Veteran Engagement Case Manager for the SSVF program. You will conduct outreach, perform intakes and needs assessments, develop service plans, coordinate housing searches, and negotiate with landlords on behalf of veterans to secure housing.

Work with a multidisciplinary team, document progress in HMIS, and participate in case conferences to ensure veterans achieve permanent housing and independence.

Qualifications

• Bachelor's degree in social work or related field or 3 years homeless services experience.
• Excellent communication skills.
• Must obtain First Aid/CPR certification within 90 days.
• California Driver License or alternate transportation ability.

Responsibilities

• Conduct outreach and collaborate with homeless providers in designated areas.
• Attend meetings, screenings, and intake assessments to determine eligibility.
• Develop individualized service plans and coordinate housing searches with landlords.
• Maintain relationships with landlords and property managers for participant housing.
• Submit housing applications and conduct inspections of potential units.
• Provide tenant education and case management, including budgeting and referrals.
• Document progress in HMIS and participate in regular case conferences.
• Deliver home-based case management and participate in on-call rotations as needed.
