Job summary

Mykroc in Pittsburg, Kansas, is seeking a Case Manager for the SSVF program to deliver case management and supportive services to Veteran households, including rapid resolution, rapid rehousing and homelessness prevention.

You will support staff training, outreach, and coordinate with VA, CoC and community partners; maintain documentation and eligibility screening to ensure timely access to services for Veterans.

Qualifications

• BA/BS is required; degree in social work preferred.
• 2–5 years of relevant experience in case management is preferred.
• Experience with Veterans or homeless services is a plus.

Responsibilities

• Assist with Staff Training: on-boarding and training for new staff.
• Outreach, Engagement, and Community Networking: build relationships with Veterans, referral sources, and landlords.
• Screening Potential Applicants: assess eligibility and prioritize referrals.
• Intake & Assessment: conduct intake, enroll participants, and educate on program requirements.
• Strengths-Based & Housing First Case Management: provide case management and coordinate housing services.
• Housing Stability Case Planning: develop and update housing stability plans with Veterans.
• Documentation: maintain real-time, accurate case files and documentation.
• Continuous Quality Improvement (CQI): track unmet needs and participate in CQI projects.
