Jewish Family Children’s Services of the Suncoast is seeking a full-time Case Manager to serve Homeless Veterans and at-risk Veterans under the Operation Military Assistance Program (OMAP). The role coordinates VA benefits, public supports, and temporary financial assistance to help clients obtain or maintain housing stability.

Qualifikationen

• Bachelor’s degree from an accredited school (preferred).
• Two years of experience working with veterans/families.
• Experience working with very low-income individuals/families.
• Ability to establish positive relationships with participants and partners while maintaining confidentiality of case material.

Aufgaben

• Make initial person-to-person contact with applicants within 24 hours.
• Determine and document each applicant’s eligibility for OMAP within 2 working days of first contact.
• Conduct an assessment of each eligible client’s needs upon acceptance into OMAP.
• Develop housing stability plan with the client during the Initial Client Visit (ICV).
• Maintain contact with each active client at least once every 10 working days until case closed.
• Enter client-specific data into HMIS in compliance with SSVF requirements.
