Job summary

The Salvation Army North and Central Illinois Division seeks a dedicated Veterans' case manager to support the SSVF program. You will conduct assessments, create housing-focused service plans, and maintain contact with Veterans to promote stability and self-sufficiency.

Responsibilities include intensive case management, referrals, documentation in HMIS, and collaboration with partners to connect Veterans with essential services.

Qualifications

• Bachelor's degree in human services or closely related field.
• One year of direct, full-time case management experience.
• Ability to work with Veterans and diverse populations using trauma-informed care.

Responsibilities

• Provide case management using CTI, Housing First, and motivational interviewing.
• Develop individualized service plans within one week of enrollment.
• Maintain contact with Veterans through visits, calls, and meetings.
• Assist Veterans in locating and maintaining permanent housing.
• Compile electronic client records and HMIS data entries.
• Coordinate intake, referrals, and linkage to community resources.
