Job summary

The Salvation Army North & Central Illinois Division seeks a dedicated SSVF Case Manager to deliver comprehensive case management and supportive services to veteran households. You will coordinate with VA, CoC, and community partners to resolve housing barriers and coordinate rapid resolution, rehousing, and prevention.

You will lead outreach, screening, intake, and housing stability planning, while documenting progress and maintaining confidentiality.

Qualifications

• BA/BS required; social work preferred.
• 2–5 years of relevant experience; MI, solution-focused interventions, and mediation.
• Case management, outreach, engagement, rapport building, conflict mediation, problem solving, and documentation.
• Proficiency in Motivational Interviewing, Crisis Intervention, or Trauma-Informed approaches.
• Travel up to 75% of the time; valid driver's license required.
• Full-time position; may require some weekend/evening work.

Responsibilities

• Provide case management interventions for Veterans with barriers.
• Coordinate with VA, CoC, mainstream resources to meet needs.
• Screen and intake: verify veteran status, income, housing needs.
• Develop housing stability plans with the Veteran household.
• Advocate for housing options and mediation with landlords.
• Perform HMIS data collection and documentation.
• Engage in outreach events and community partnerships.
