Job summary

The Salvation Army North & Central Illinois Division is seeking an SSVF Case Manager in Danville, IL. You will provide intensive case management to Veterans and their families, helping to secure permanent housing and access community resources.

The role emphasizes CTI, Housing First, and trauma-informed approaches while maintaining thorough client records and collaborating with local partners. Requirements include a bachelor’s degree in human services or related field, 1 year of case management

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• One year of direct, full-time case management experience working with the homeless population and/or Veterans
• Valid Driver’s license/MVR clearance
• Experience with Critical Time Intervention (CTI), Harm Reduction, Housing First, Trauma-Informed Care, Motivational Interviewing and conflict resolution preferred
• Excellent written and verbal communication skills; ability to work with diverse populations
• Willing to travel regularly to meet Veterans and partners

Responsibilities

• Case Management (70%): Provide intensive case management using CTI, Housing First, Strength-Based approaches, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction.
• Develop individualized service plans within one week of enrollment with measurable short- and long-term goals, timelines, and action steps.
• Maintain regular contact with enrolled Veterans through office/home visits, calls, and community meetings.
• Conduct assessments to identify barriers to housing, income, employment, health care, and community integration.
• Assist Veterans in locating, securing, and maintaining permanent housing and conducting housing inspections.
• Assist Veterans with intake and referral needs, including SSDI/SSI benefits through SOAR.
