Job summary

The Salvation Army North and Central Illinois Division is seeking an SSVF Case Manager in Danville, IL. This role supports Veterans and their families with intensive, strengths-based case management to achieve housing stability under the SSVF program.

The position reports to the SSVF Region 3 Supervisor and involves collaboration with VA and community partners, including outreach, intake, and ongoing client support.

Qualifications

• Bachelor's degree in human services, social work, or a closely related field.
• One year of direct, full-time case management experience working with the homeless population and/or Veterans.
• Valid Driver's license/MVR clearance.

Responsibilities

• Case management across traditional and Shallow Subsidy SSVF programs.
• Provide intensive case management using CTI, Housing First, Strength-Based approaches, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction principles.
• Develop individualized service plans within one week of enrollment with specific measurable, short- and long-term goals, timelines, and action steps designed to promote housing stability and self-sufficiency.
• Coordinate regular contact with enrolled Veterans through visits, calls, and community-based meetings to support housing stability.
