Job summary

Columbus House, Inc. seeks a full-time SSVF Case Manager (32 hours/week), non-exempt, to serve veteran households at risk of homelessness in New Haven, CT.

Under the SSVF Program Manager, you will provide housing search, counseling, service coordination, and progress evaluation. Qualifications include a bachelor’s degree in human services and at least two years’ experience, strong organizational and communication skills, proficiency with Microsoft Office, and a valid driver’s license.

Qualifications

• Bachelor’s degree in human services required.
• Two years’ experience in human services or related field.
• Excellent organizational and time management skills.
• Strong written and oral communication skills in English.
• Valid driver’s license with good driving history.

Responsibilities

• Provide case management services to veteran households at risk of homelessness.
• Assist housing search and placement within 90 days of program entry.
• Provide linkage to social, mental health, educational and other services.
• Coordinate with other agencies and attend weekly meetings.
• Maintain all client charts in the appropriate system and complete documentation.
• Manage a 30-client caseload with timely charting and reporting.
• Monitor progress toward housing goals and program outcomes.
