Job summary

U.S. VETS is seeking a Case Manager II to support veterans facing housing instability, trauma, and mental health challenges. The role requires a bachelor’s degree in social work, psychology, counseling, or related fields, plus hands-on case management experience.

You will collaborate with VA, landlords, employers, and service providers, conduct assessments, develop service plans, and lead life skills groups for diverse veteran populations.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• At least two years of work experience in case management or other roles in social services.
• Ability to pass background checks for programs involving incarcerated or recently released clients.
• Strong communication, documentation, and organizational skills.
• Proficiency in Microsoft Office and electronic records systems.
• Valid and current driver’s license with clean driving record; ability to complete driver training.

Responsibilities

• Provide trauma-informed, strengths-based case management to veterans.
• Coordinate and monitor services with VA, landlords, employers, treatment providers, and community resources.
• Conduct regular one-on-one meetings with clients and home visits (project-based or scattered-site).
• Transport clients in company vehicles to appointments and meetings as needed.
• Facilitate life skills or therapeutic groups for veteran populations.
• Assist with crisis intervention and after-hours support when needed.
• Document all services and critical incidents in required systems.
