Job summary

American GI Forum - National Veterans Outreach Program in Fort Worth, TX seeks a Case Manager for the SSVF program. You will develop plans with clients to stabilize at-risk veteran families and coordinate housing placement, financial assistance, and access to VA benefits and healthcare.

The role requires a Bachelor's or Associate degree with relevant experience, a valid Texas driver’s license, and the ability to navigate internal and external service providers.

Qualifications

• Bachelor's Degree in a related field and 3 years’ experience with low-income families or veterans preferred.
• Or an Associate’s Degree with at least 5 years’ related work experience.
• Must have a valid Texas driver’s license and transportation.
• Proof of vehicle liability insurance.
• Must pass a drug test and background check.

Responsibilities

• Develop a case management plan with clients to assess needs and barriers.
• Provide housing placement assistance and temporary financial support to stabilize families.
• Coordinate VA benefits, healthcare, and other services for clients.
• Prepare and submit MIS data and service requests as needed.
• Maintain client files and documentation, including follow-ups and correspondence.
