Job summary

Qualifications

• BA/BS required; social work preferred.
• 2–5 years of relevant experience in case management/ homeless services.
• Experience with motivational interviewing, crisis intervention, and trauma-informed approaches.
• Strong communication and documentation skills.

Responsibilities

• Provide case management and supportive services to Veteran households.
• Assist with outreach, engagement, referrals, and housing navigation.
• Coordinate with VA, CoC, mainstream, community-based, and legal resources to meet needs.
• Maintain real-time, accurate case file documentation and confidentiality; HMIS entry.
• Travel up to 75% of the time; field and office work as needed.
