Job summary

Volunteers of America Chesapeake & Carolinas seeks a Case Manager to provide day-to-day support for veteran households within the SSVF program. You will help with intake, assess needs, and connect clients to housing and wraparound services.

Qualifications

• Bachelor's degree in counseling psychology, rehabilitative counseling, or related human sciences.

Responsibilities

• Coordinate outreach to VA, shelters, and other housing providers.
• Screen program participants for eligibility using VA-approved forms.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners.
• Travel to housing sites to support participants and address health or life skills issues.
