Job summary

Appalachian Agency for Senior Citizens in Wyoming seeks a Community Health Coordinator to educate Veterans about the Veterans Directed Care program and coach them in creating personalized Spending Plans. The role blends home visits with office duties, working to coordinate services and monitor progress with the Veteran and the FMS provider.

Must have a Bachelor’s degree (or equivalent), 3+ years serving seniors or related fields, and a valid driver’s license.

Qualifications

• Bachelor's degree in social work, human services or medical field or equivalent experience preferred.
• 3+ years of experience with seniors, disabled adults, human services or case management preferred.
• Criminal history background check required.
• Valid driver's license and proof of current PPD test required.

Responsibilities

• Conduct home visits and develop spending plans for Veterans in VDC.
• Office work: follow up on referrals, maintain confidential client records, and handle fiscal duties.
• Coordinate between programs and potential clients; provide liaison services.
