Qualifications

• 3+ years of experience working with seniors, disabled adults, human services or general case management.
• Criminal history background check required.
• Annual recertification is required.

Responsibilities

• Provide person-centered counseling to Veterans in the Veterans Directed Care program.
• Develop spending plans with Veterans and assist in becoming an employer.
• Monitor and coordinate supports and services and track expenditures.
