Job summary

Western Reserve Area Agency on Aging is seeking a Veteran-Directed Care Manager to provide person-centered counseling, care coordination, and ongoing support to Veterans enrolled in the VDC program. You will partner with Veterans, ARs, caregivers, VAMC staff, and FMS to develop and monitor spending plans that align with the Veteran's goals and independence.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, Nursing, Rehabilitation Counseling, or related field.
• Minimum of two years of experience in case management, care coordination, social services, aging services, veterans services, healthcare, or related field preferred.
• Licensed Social Worker (LSW), LISW, Registered Nurse (RN), or other relevant credential preferred but not required.

Responsibilities

• Provide person-centered counseling and care coordination to Veterans enrolled in the VDC program.
• Develop and monitor individualized spending plans in collaboration with Veterans, Authorized Representatives, caregivers, and VAMC staff.
• Maintain regular contact with Veterans via home visits, telephone calls, virtual meetings, and other approved methods.
• Ensure compliance with VA regulations, Veterans Directed Care requirements, agency policies, confidentiality standards, and documentation expectations.
• Advocate for Veteran choice, independence, dignity, and community inclusion.
