Job summary

Bay Aging is seeking an Options Counselor to provide person-centered counseling to Veterans enrolled in the Veterans Directed Care program. You will help Veterans identify needs, develop spending plans, and coordinate supports with the VDC and FMS providers.

The role includes extensive home visits in Maryland with travel, plus office follow-up, scheduling, and documentation. A bachelor’s degree and 3+ years’ experience in human services are preferred.

Qualifications

• Bachelor’s degree in social work, human services or medical field preferred.
• 3+ years of experience with seniors, disabled adults, human services or case management.
• Strong Microsoft Office skills and data entry ability.
• Excellent verbal and written communication and customer service.
• Ability to travel regularly and work independently.

Responsibilities

• Provide person-centered counseling to Veterans enrolled in the Veterans Directed Care program.
• Conduct home assessments and develop spending plans with Veterans and their families.
• Coordinate supports with the Veterans Directed Care program, FMS provider and VAMC.
• Maintain records and prepare monthly reports; travel to Veteran homes as required.
• Monitor expenditures and reassess needs on an ongoing basis.
