Job summary

Appalachian Agency for Senior Citizens is seeking a Community Health Coordinator to educate Veterans in the Veteran-Directed Care program and coordinate spending plans. The role applies advocacy and coaching to help Veterans identify needs and access supports.

The position involves home-based assessments, travel, and collaboration with the Financial Management Service and VAMC to monitor spending and program outcomes, while maintaining strict confidentiality and professional ethics.

Qualifications

• Bachelor’s degree in social work, human services or medical field, or equivalent experience.
• Minimum 3+ years of experience with seniors or disabled adults.
• Criminal history background check required; valid driver’s license and PPD test.
• Proficiency with HIPAA rules and Microsoft Office.

Responsibilities

• Conduct home visits and develop spending plans with Veterans.
• Perform office duties: referrals, records, and monthly reports.
• Coordinate care between agency programs and clients.
• Maintain confidentiality and perform other agency duties.
• Travel locally and engage with community partners.
