Job summary

Western Reserve Area Agency on Aging seeks a Veteran-Directed Care Manager to provide person-centered counseling, care coordination, and ongoing support to Veterans enrolled in VDC. You will partner with Veterans, Authorized Representatives, caregivers, VAMC staff, and community providers to develop spending plans that reflect goals and independence.

Qualifications

• Bachelor’s degree as listed and licensure/certification preferred but not required.
• Experience in case management, care coordination, social or aging services is preferred.
• Strong communication and organizational abilities.

Responsibilities

• Provide person-centered counseling, care coordination, and ongoing support to Veterans in the VDC program.
• Develop and monitor individualized spending plans and assist with decision-making and vendor coordination.
• Maintain accurate documentation and ensure VA compliance and program requirements.
