Job summary

Volunteers of America Colorado seeks a Health Care Navigator to connect Veterans with VA and community health benefits, coordinate care, and provide education. The role supports the SSVF program with an interdisciplinary team and outreach across service areas.

Primary duties include non-clinical assessments, care coordination, and resources to help Veterans meet health goals. Travel within service areas and strong communication are essential to success.

Qualifications

• Bachelor’s degree in human services, social work, or related field with 1 year direct case management experience.
• Master’s degree in social work, counseling, or related field.
• Colorado driver’s license and state-mandated automobile insurance.

Responsibilities

• Coordinate referrals to VA and community health clinics to ensure access to care.
• Document calls and intake, maintain care plan goals and progress.
• Advocate for Veterans, integrate cultural values into care plans.
• Provide case management and health education to Veterans and families.
• Engage in outreach to inform eligible clients of services and community resources.
