Job summary

Southwestern Community Action Council, Inc. seeks a Service Coordinator/Case Manager for the SSVF program in Beckley, WV. This full-time, non-exempt role supports veteran families with case management, outreach, and referrals to VA benefits.

The position requires a high school diploma or GED, knowledge of community resources, and strong communication. A valid driver’s license and ability to pass a drug screen are required.

Qualifications

• H.S. Diploma/GED.
• Thorough knowledge of community resources to assist clients.
• Ability to complete tasks with little direct supervision.
• Excellent written and oral communication skills.
• Skills and knowledge to maintain a professional office environment.
• Ability to maintain thorough and organized records.
• Current valid driver’s license with safe driving record.
• Must pass drug screen and background check.

Responsibilities

• Work with veterans through the SSVF program to lift very low-income Veteran Families out of homelessness.
• Assist clients in accessing community resources and public benefits.
• Maintain thorough records using the master client database and timely updates.
• Provide outreach to local resources and explore partnership opportunities.
• Collaborate with other programs to coordinate applications and referrals on a case-by-case basis.
• Provide strength-based case management and support in employment, transportation, housing, and crisis situations.
