Qualifications

Responsibilities

• Assess and diagnose patients at Oak Street Health clinics.
• Oversee, direct, and administer primary care within a team-based model.
• Prescribe and manage medications and therapies for Medicare patients.
• Maintain patient records in EMR systems.
• Collaborate with leadership to meet quality and outcomes goals.
• Lead center care team with Practice Managers.
