Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare patients within our neighborhood centers. Providers work with large teams to focus on quality outcomes rather than volume of services.

Qualifications

• Must hold an MD or DO degree and be licensed to practice in the United States.
• Board certification or eligibility in Internal Medicine or Family Medicine is required.
• Active, unrestricted State License and active DEA license are required.
• US work authorization is required to work in the United States.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications and treatments.
• Maintain patient electronic medical records.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct the care team.
