Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based care to local Medicare populations. You will work in a neighborhood primary care center with a care team to deliver high-quality outcomes and an unmatched patient experience.

Responsibilities include diagnosing patients, overseeing primary care delivery, prescribing medications, and maintaining EMR data. Board-certified MD/DO with US license and DEA is required; geriatrics training is a plus.

Qualifications

• MD or DO graduate.
• Internal Medicine or Family Medicine board certification (or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
