Job summary

Oak Street Health in Philadelphia is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare. You will see patients independently within a collaborative care team, conducting visits, wellness exams, and care coordination.

Requires MSN/NP, board certification, active license, and DEA; full-time, 40 hours weekly; competitive benefits and base pay between $132,825 and $165,925 per year, with a comprehensive benefits package.

Qualifications

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
National certification in Family NP, Adult-Gerontology, Adult NP or Gerontological NP.
Active, non-probationary state NP license and Active DEA license.
US work authorization.

Responsibilities

Provision of exceptional primary care.
Conduct office visits for routine and acute issues.
Administer annual wellness visits and health risk assessments with accurate documentation.
Care coordination with providers, specialists, testing facilities, and agencies.
Population health leadership in coordination with the Care Team.
Assist with phone triage and outreach.
Educate patients on health conditions and care plans.
Participate in Oak Street Health promotional activities.
Conduct home visits as needed.
Other duties as assigned.
