Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, value‑based primary care to adults on Medicare in outpatient clinics. You will work with a collaborative care team to provide longitudinal care, perform wellness visits, and coordinate with physicians, specialists, and care managers to help patients stay healthy and out of the hospital.

The role requires MS in Nursing or NP credentialing, national certification in required specialties, an active NP license and US work

Qualifications

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
Active, non-probationary state NP license.
Active DEA license.
US Work Authorization.

Responsibilities

Provision of exceptional primary care.
Conduct office visits for routine and acute issues.
Administer Annual wellness visits and health risk assessments, with holistic health documentation.
Care coordination with providers, specialists, testing facilities, and agencies.
Population health leadership in coordination with the Care Team.
Assisting the care team with phone triage and outreach.
Educating patients on health conditions, care plans and treatments.
Participating in Oak Street Health promotional activities.
Conducting home visits as needed.
