Job summary

Oak Street Health in the United States seeks a Nurse Practitioner, Advanced Practice Provider, to deliver effective, equitable value‑based primary care to adults on Medicare, keeping patients happy, healthy, and out of the hospital. NPs see patients independently and collaborate with physicians depending on the case.

Outpatient practice teams include a physician, NP or PA, medical assistants, a scribe, RNs and care managers.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care to adults in a value-based model.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, testing facilities, and care teams.
• Lead population health initiatives in coordination with the care team.
