Job summary

Oak Street Health in the United States is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare. You will work on a collaborative care team within outpatient clinics, conducting visits, wellness checks, and care coordination to keep patients healthy and out of the hospital.

Qualifications

• MSN, DNP, or NP certificate program graduate.
• Active, non-probationary NP license in the state.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health initiatives with the care team.
• Assist with phone triage and outreach.
• Educate patients on health conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
