Oak Street Health is seeking a Nurse Practitioner, Advanced Practice Provider to deliver value-based primary care to adults on Medicare. The NP will work on a collaborative care team to provide longitudinal care and coordinate with physicians.

Responsibilities include wellness visits, comprehensive documentation, care coordination, and population health initiatives. Requires NP degree, active state license, and DEA license; US work authorization is required. Full-time; base pay range applies.

Qualifications

• MSN/ DNP or NP certificate graduate is required.
• Active NP license in state of practice is required.
• Active DEA license.
• US work authorization is required.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments.
• Care coordination with physicians, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the care team.
